Normally a range of services contribute to and support the provision of intermediate care; some are well established services who contribute to intermediate care aims and some are specialist services, whose reason for existing is intermediate care in a particular place or setting or for a particular group of patients or people e.g. stroke
It is hoped local developments within Denbighshire will ensure that there is effective communication between a network of services which provide intermediate care to ensure a positive experience for patients and service users.
Over the next few months, services which can be defined as delivering intermediate care or support intermediate care will be identified on this page as forming part of the Intermediate Care Network in Denbighshire.
The various tiers of the intermediate care network might be described as follows:
