Your concerns will be listened to and we will aim to rectify them straight away. If the concern is regarded as a complaint, the information will be passed to the relevant organisation to manage, with your permission.

Any suggestions will be logged and considered when further planning services.

It is hoped to develop an evaluation form which will be left with you on discharge from an intermediate care service.

If you would like to become involved in the development or evaluation of the services, whether or not you have received intermediate care, you are welcome to do so either by:

  • Phoning the Intermediate Care Manager as above with your ideas and comments or
  • Contacting a Voluntary Sector Organisation who will talk to you and then contact the service on your behalf – anonymously if required.
  • Becoming a member of an Intermediate Care reference group which will shortly be formed – To join, please contact the Intermediate Care Manager as above or

  • Becoming a member of existing planning forums who will pass on information to the services. These groups may include:
