Who Was Involved?

Learning Disabilities

Adults services are currently separately managed by health and social services. Although both teams are co-located and have a history of collaborative working, this project would allow work to progress to the next level of joint working.

Recent efforts to progress joint working have not been successful. The main reason was considered to be the lack of capacity to dedicate time that the project deserves

The proposal aims were to address concerns by the employment of a Project Development Manager to facilitate closer integration in the planning and delivery of service including transition Children to Adult's services.

 

Learning Disabilities - Adults

It is proposed that the project will:

  • Firmly re-establish joint service provision as a priority within Learning Disability Services.
  • Enhance current working relationships between local health and social care teams.
  • Reduce duplication in current service planning and provision enabling resources to be deployed more effectively
  • Ensure a positive step forward is taken towards closer integration e.g. shared strategic planning.
  • Enhance active service user and carer involvement in the planning, provision and monitoring of service.

 

Children with Disabilities

  • To work on unifying the Eligibility Criteria of the various services young people come to contact with.
  • To set up a sub group to discuss what criteria is being used, what parameters services are working with and map these to provide a findings report.

Identifying and reviewing the needs of:

  • Children with disabilities and their families.
  • Adults with learning disabilities.
  • Effective consultation and participation.
  • Co-ordinated planning and provision within and across the age groups.
  • Learning from and building on good practice.
