Transition

To develop a transition service from Children to Adults services that is well planned and coordinated by:

• Identify gaps in services

• Develop Joint Guidelines

• Develop Joint Protocol / System

• Develop a Joint Flowchart

A sub-group was formed, and produced a flowchart and developed a joint protocol that will be able to guide service users, parents, carers and professionals through the difficult phase of transition.

Three leaflets have also been produced that will be distributed at various stages during transition.

2. A directory of services with contact details
3. A service user guide - which is easy read
