How can I access intermediate care services?
You may find the intermediate care services helping you if you are admitted to or are likely to be admitted to hospital or a care home. You may have an initial assessment whilst in a community hospital or residential home or you may be referred to the service by any health or social care professional such as a social worker or GP with your approval
If you feel you have health and social care needs and could benefit from intermediate care then you can make contact with the First Contact Team in Social Services who will ask you some simple questions, after which you may be referred to an Intermediate Care Team or a different team who could more appropriately help you.
