Project Details
The proposed Intermediate Care model for Denbighshire comprises of three incremental phases developed over a three year time period, delivered through a partnership of the Denbighshire Local Health Board, Conwy and Denbighshire NHS Trust and Denbighshire Social Services.

An Intermediate Care Project Board has been set up whose primary role is to discuss proposals and agree plans for an integrated network of Intermediate Care services for the adult residents of Denbighshire. The Project Board will be supported and advised by the Intermediate Care Manager, who will in turn be supported by the Intermediate Care Programme Management Group, members of which will have operational and commissioning responsibilities.
